What this is
A site about one narrow thing: how a care plan gets finished once somebody has one.
Where it sits
Households navigating care run into three separate questions, and they are genuinely different questions rather than one question split by who answers it.

What are we entitled to
A statutory question. There are rules, the rules are written down, and either somebody meets them or they do not. The answer is frequently unknown, and unknown must never be presented as a no.

What is actually open near us
A local question with capacity attached. A service that exists on paper and has a nine month waiting list is a different answer from one that can take somebody next week.

What happens next, and who is doing it
Neither statutory nor capacity-limited. This one is about execution, and it is the question this property is about.
The free check that answers the first two, and produces the plan, is at AIOSeniors. A household completing it gets one plan and never sees the seam between the parts that produced it.
What this site does not do
- It does not decide whether anybody qualifies for anything. The agency running a program makes that determination. What can honestly be said is what a rule says and the date it was checked.
- It gives no medical, legal, financial or insurance advice. Practical navigation only.
- It promises no outcome. Nothing here makes an application more likely to be approved. It makes it more likely to be sent.
- It sells nothing. There is no price anywhere on this site, on purpose.
Who built it
Digilu. The same company builds the free check that produces the plan and the ongoing coordination product that this site points to, which is worth stating plainly rather than leaving somebody to discover: when this site says the permanent side of the problem needs Family Observatory, that is a recommendation with a commercial interest behind it.
Which is exactly why the recommendation is evidence-gated. A household whose plan finished cleanly, with the work shared, is told that nothing here suggests they need anything else, and nothing is offered to them. How that gate works.
Independence
AIOCoordination is not a government agency, not an insurer, and not affiliated with Medicare, Medicaid, the Social Security Administration, the Department of Veterans Affairs or any state agency. Where a program is described, the official source is the authority and we are not.

The part that does not end
One plan finishes. The driving, the meals and the appointments do not.
Follow-through closes out a single plan and then stops, on purpose. The ongoing work of a family sharing care is a different job: who is taking him on Thursday, who is picking up dinner, who is quietly doing all of it. That is what Family Observatory is for.
